I give N-Tune Music and Sound, Inc. permission to bill my monthly rental payment to the credit/debit card listed below. I agree that the monthly charge will be applied to my credit/debit card according to my N-Tune Music Rental Agreement, and in order to cancel the recurring billing process, I am required to contact N-Tune Music and Sound, Inc. in writing, one (1) month prior to the next scheduled debit to either cancel the agreement by return of merchandise or arrange for alternate method of payment. I will provide new card information when my card expires or my card number changes. I understand that N-Tune Music and Sound, Inc. will not mail me any invoices or bills. Should the account become delinquent, the past due balance will be charged to the card on file. N-Tune Music and Sound Inc. does not have to send me a notice of the debit amount if the amount of the debit is between $0.00 and $500.00. I agree that if I have any problems or questions regarding my account or any services provided by N-Tune Music and Sound, Inc. I will contact N-Tune Music for assistance using the contact information provided on my Rental Agreement. I also agree that I will not dispute any charges with my credit card company or Banking institution without first making a good faith effort to remedy the situation directly with N-Tune Music and Sound, Inc.

I guarantee and warrant that I am the legal card holder for this credit/debit card and that I am legally authorized to enter into this recurring credit/debit card billing agreement with N-Tune Music and Sound, Inc.

Changes to banking or card information must be received 30 days prior to the next scheduled due date. N-Tune Music is not liable for any additional fees that are incurred as a result of banking changes that are not received at least 30 days prior to the next scheduled due date (we will not pay overdraft fees that your bank charges you.) Payments that are returned for any reason will be resubmitted with an additional fee of $30.00. Should the account fall past due, the past due balance, including all fees, will be drafted. This authorization shall remain in full force until I cancel it in writing by sending my notice at least thirty (30) days prior to the due date of the next scheduled debit and return the instrument to: N-Tune Music and Sound, Inc. 1141 E. 42nd Street Odessa, Texas 79762 I understand that it is my responsibility to return the instrument to N-Tune Music and Sound, Inc. and NOT to the music instructor or school.
